Parent Consent & Liability Form

I am the parent or legal guardian of the child listed below (“Participant”). I give my permission for my child to participate in the Junior Golf Instruction Program (“Program”) offered by Langdon Farms Golf Club (“Facility”).

Assumption of Risk
I understand that golf instruction and related activities involve inherent risks. These may include, but are not limited to, being struck by golf balls or clubs, slips or falls, uneven terrain, weather conditions, physical exertion, and use of golf equipment. While the Facility takes reasonable steps to provide a safe environment, injuries can occur. I knowingly and voluntarily assume all such risks on behalf of myself and the Participant.

Release of Liability
In consideration for my child’s participation in the Program, I agree to release, waive, and discharge the Facility and its owners, employees, instructors, coaches, volunteers, agents, and affiliates (“Released Parties”) from any and all claims or liability arising out of my child’s participation in the Program, to the fullest extent permitted by law.

Indemnification
I agree to indemnify and hold harmless the Released Parties from any claims, damages, or expenses (including reasonable attorneys’ fees) resulting from my child’s participation in the Program or my failure to comply with this Agreement.

Medical Authorization
I confirm that my child is physically able to participate in the Program and that I have disclosed any relevant medical conditions to the Facility. In the event of an emergency, I authorize the Facility to obtain medical treatment for my child if necessary and agree to be responsible for any associated costs.

Media Release
I grant permission to the Facility to photograph and/or record my child during Program activities and to use such photos or video for promotional, marketing, educational, or informational purposes, including use on the Facility’s website, social media, and printed materials.

Code of Conduct
I understand that all participants are expected to behave in a respectful and safe manner. This includes following directions from instructors and staff, respecting other participants and Facility property, and complying with all Facility rules and policies. I acknowledge that failure to follow these expectations may result in my child’s removal from the Program without refund.

Acknowledgment
I have read and understand this Agreement and sign it voluntarily. I understand that it is binding upon me, my child, and our heirs or legal representatives. If any portion of this Agreement is found unenforceable, the remaining sections shall remain in full force and effect.
